Effect of Occupational Complexity, a Proxy for Cognitive Reserve, on Alzheimer’s Disease Risk: A Systematic Review

ABSTRACT 
Alzheimer’s disease increasingly burdens aging societies, affecting over 32 million globally. Literature inconsistently addresses the relationship between occupational complexity on cognitive reserve and hence Alzheimer’s disease risk. PubMed and Semantic Scholar databases were used in this systematic review, adhering to the PRISMA guidelines to assess the impact of occupational complexity on cognitive reserve. Of the 5,208 articles yielded, 36 satisfied the inclusion criteria; 12 of which examined the link between occupational complexity and Alzheimer’s disease, while the 24 examined other characteristics based on cognitive reserve. Greater occupational complexity and education is associated with higher cognitive reserve, delaying Alzheimer’s onset and maintaining function, but caused cognitive decline post-diagnosis. Higher education and white-collar jobs provided neuroprotection, whereas blue-collar and low socioeconomic position increased risk. Cognitive reserve advantages differed by gender and ethnicity, highlighting the inequalities in access to cognitive enrichment. Longitudinal investigations with thorough cognitive reserve evaluation are required to deepen understanding. These results present the necessity of fair access to intellectually enriching education and work since cognitive reserve improvement could be a key measure in decreasing the burden of Alzheimer’s disease on the global scale.
